A T4A slip is a statement of  benefits (e.g. Basic Life, Accidental Death and Dismemberment). This is issued by OTIP, the plan administrator of your ETFO Employee Life and Health Trust Benefits Plan.  If  you have Basic Life and Basic Accidental Death and Dismemberment (AD&D) and are actively at work, those benefits are paid for by your Employee Life and Health Trust (ELHT).  If you had this coverage at any time between January 1 – December 31, 2022, you will be issued a T4A slip for these taxable benefits if the taxable amount is more than $25.
