Please note that, as communicated by OTIP on February 5th, T4A will only be available digitally beginning with the 2023 T4A.  

A T4A slip is a statement of  benefits (e.g. Basic Life, Accidental Death and Dismemberment). This is issued by OTIP, the plan administrator of your ETFO Employee Life and Health Trust Benefits Plan.  If  you have Basic Life and Accidental Death and Dismemberment (AD&D) and are actively at work, those benefits are paid for by your Employee Life and Health Trust (ELHT).  If you had this coverage at any time between January 1 – December 31, 2023, you will be issued a T4A slip for these taxable benefits if the taxable amount is more than $25.  Click here for more information about your T4A and how to access it on the OTIP member secure site, or visit OTIP’s T4A FAQ on their website.
